Four stages across your first year.

Each stage steps down from the one before. Case management and peer support stay with you the whole way.

  1. Stage 1 · PHP

    Partial hospitalization

    25 hours a week

    Group therapy, a weekly individual session, and a nurse practitioner and RN nursing staff on hand. The average stay is 2–4 weeks.

Stage 1

Partial hospitalization (PHP)

What happens
  • Structured group therapy
  • One individual therapy session a week
  • A nurse practitioner and RN nursing staff, who complete intake assessments and are there for ongoing clinical needs
  • Case management and peer support
  • A curriculum covering essential recovery topics, at a structured pace

2–4 weeks, on average
Who it’s for
Men who need the most structure: stabilizing after residential treatment, or coming straight from the community without needing detox first.

Need detox first? Reclaim doesn’t offer detox. We can refer you to local partners for safe medical detox before treatment starts.

Stage 2

Intensive outpatient (IOP)

12 hours of group therapy a week, over 4 days

What happens
  • Group therapy 4 days a week, including holistic healing sessions
  • Individual therapy every other week
  • Case management and peer support
  • A curriculum on where addiction comes from, managing cravings, recognizing triggers and preventing relapse

Men stepping down from PHP or residential treatment, and men who need steady structure while keeping up with daily routines and responsibilities.

Outpatient (OP)

1–2 hours of group therapy, one day a week

Reclaim Recovery Community

Addiction and mental health, treated together.

Addiction can bring on mental health struggles like depression, anxiety and trauma. Here, both are treated together, with therapy, counseling and holistic methods.

  • Individual and group therapy with licensed and certified clinicians: LCSW, LCADC, LPCA and CADC
  • Coordination with your prescribing providers for medication management, including medication-assisted treatment (MAT) and serious mental illness (SMI) support, at every level of care
  • Referrals to psychiatric specialists when needed

In IOP, the clinical groups include holistic healing sessions.

Where will I start?

Your intake assessment helps set the level that fits. Our nurse practitioner and RN nursing staff complete intake assessments.

You start where you need the most structure, then step down as you’re ready.

Do you offer detox?

No. If you need medical detox first, we can refer you to local partners for safe medical detox before you start treatment.

Can I keep working?

IOP and outpatient are flexible, so you can keep up with your daily routines and responsibilities while you attend.

PHP is the most structured level, at 25 hours a week. Ask admissions how the hours fit around your job.

What about MAT?

Every level of care coordinates with your prescribing providers for medication management. That includes medication-assisted treatment (MAT) and support for serious mental illness (SMI).

Do I need a referral?

No referral is required. You can call admissions yourself, and so can a family member.

Our intake staff can also complete the assessments that Medicaid or a court may require.

What does it cost?

With Kentucky Medicaid, there are no fees for treatment. Without it, admissions can help you apply for benefits, and there are flexible payment plans if you don’t qualify.

Can my family help?

Yes. The family program runs monthly educational classes, with family therapy as needed, based on each client’s treatment plan.

How long does it last?

The program is built around a full year of care. Stays average 2–4 weeks in PHP and about 50 days in IOP.

Outpatient and the Reclaim Recovery Community then carry on through the rest of your first year.
